We are a growing mobile wound care practice dedicated to delivering compassionate, evidence-based treatment to patients wherever they call home. Our patients are seen across diverse settings — private residences, skilled nursing facilities (SNFs), board-and-care homes, and assisted living communities.

Position Summary
We are seeking a Registered Nurse or LVN with a passion for wound management and patient-centered care. The ideal candidate is reliable, compassionate, and able to work independently while maintaining strong communication and coordination with our interdisciplinary team.

Key Responsibilities

  • Provide wound assessment, treatment, and follow-up in home and facility settings
  • Communicate and coordinate care plans with physicians, facility staff, and caregivers
  • Maintain accurate and timely documentation in the EMR
  • Collaborate with case managers and other healthcare professionals to ensure continuity of care
  • Drive between patient locations within the assigned region

Qualifications

  • Current and active California RN license or California LVN license
  • Reliable transportation and willingness to travel between patient locations
  • Strong clinical assessment and bedside skills
  • Excellent verbal and written communication abilities
  • Comfortable using electronic medical record (EMR) systems
  • New graduates are welcome – must be eager to learn, coachable, and committed to professional growth

Preferred Qualifications

  • Experience in wound care, home health, hospice, or post-acute settings (preferred but not required)
  • Wound care certification (WCC, CWCN, CWS, or similar) a plus
