Hospice PRN On Call RN

Rivercross Hospice is hiring for a PRN On Call RN in the Kingman/Pratt area!

The Rivercross Hospice improves the quality of life of patients and their families facing the problems associated with progressive advanced illness, through the prevention and relief of suffering.

The RN will provide outstanding care to patients with Advanced Illnesses. This care is based upon the nursing process; assessment, nursing diagnosis, planning, implementation and evaluation. The On Call RN must also provide excellent communication and coordination of care with patient’s primary care provider, other Palliative Team members and miscellaneous community providers. Stellar clinical documentation is required. Actively supports and promotes the hospice’s philosophy and mission statement and consistently monitors patients for decline.

Primary Responsibilities:

• Coordinate with the RN Manager to assure the client driven plan of care that strives to meet positive outcomes through on-going evaluation using evidenced based practice
• Responsible for making certain that hospice care provided is compliant with State and Federal regulations (documentation and delivered) through participation in chart review and following established process
• Responsible for participation in orientation, ongoing education programs, and successful completion of competency requirements
• Provide services to meet both internal and external customer's expectations

Required Licensure and Certification:

• Current/Active RN License
• Current CPR certification.
• Must also be a licensed driver and possess an automobile that is insured in accordance with state and/or organizational requirements and is in good working order. Must maintain a clean driving record

Pay: $35.00 - $40.00 per hour
